WAIVER

With the acceptance of this application and the permission given to have my child, named above, participate in any dance or dance related activity with the Born to Move Dance Company (B2M Dance Co.) and Canada’s National Academy of Dance Education. I hereby release, waive and forever discharge the Born to Move Dance Company, Canada’s National Academy of Dance Education and all of their members against all claims (i.e. liability, damages, costs etc.), howsoever caused, arising as a result of or in any way connected with my child’s participation in any dance or dance related activity. By submitting this registration form, I acknowledge having read, understood and agreed to the above waiver, release and indemnity. I warrant that my child is physically fit to participate in such activity and acknowledge that I am aware that there are risks associated with it which I hereby agree to entirely assume as my own personal responsibility. I understand the physical risks involved in this activity and I do not hold the Born to Move Dance Company or Canada’s National Academy of Dance Education accountable for any injuries that may occur due to (child named above) participation in this event/activity.


ELEMENTS OF RISK

The Born to Move Dance Company and Canada's National Academy of Dance Education programs, being offered, involves certain elements of risk. Accidents may occur while participating in this activity. These accidents may cause injury. A few examples of the type of injuries which one is at risk of having occur while participating in this activity are:

Activity related (e.g., minor cuts and abrasions, sprains and strains, dislocations and fractures, to more serious injuries affecting the body. Some head, neck, or back injuries could lead to paralysis or prove to be life threatening.)

These injuries result from the nature of the activity and can occur without any fault on either the part of the dancer, or the Born to Move Dance Company or Canada's National Academy of Dance Education, its affiliates, its employees or agents, or the facility where the activity is taking place. By choosing to participate in the activity, you are assuming the risk of an injury occurring.

Carefully following instructions at all times and being physically fit to participate in the activity can reduce the chance of an injury/accident occurring.


PHOTO RELEASE

I hereby authorize Canada’s National Academy of Dance Education/Born to Move Dance Company, and its clients, to publish the photographs taken of my minor child, in which he/she may be included with others, including the surrounding areas depicted in said photographs, for use in any and all media, printed publications, marketing, websites and digital programs/forms/publications without restriction for any private or public purpose.

I hereby release both the Canada’s National Academy of Dance Education/Born to Move Dance Company, its contractors, clients, employees, officers, legal heirs, investors, agents, representatives, and assigns from all liability for any claims by me, my minor child, my family or any third party in connection with my child’s participation, including any claims and demands ensuing from or in connection with the use of the photographs, including any and all claims for libel and invasion of privacy. I also release them from any expectation of confidentiality for the use of said photographs.



TERMS & CONDITIONS

Parents please carefully review each item as it contains important information about your experience with us at the Born 2 Move Dance Company. Initial beside each item. PAYMENTS

If paying month to month, fees are due on the 1st of each month (Sept-May) via Credit Card Pre-Authorization. If your credit card is expired or declined, there will be a $30 administrative fee added on to the monthly payment for that month. Please note that if your account is not up to date, we will refuse admission into class/competition/recital, and will not release recital costumes or tickets until fees are paid and the account is in good standing.

All one lump sum payments and monthly payment schedules include 9 Months of Equal Payments Sept-May.

Please note that all prices listed online, on our website or in print/media ads do not include tax.

NSF Fee of $30 applies to all returned declined payments, expired credit cards etc.

All monthly fees, when paying month to month, are subject to a $3.00 monthly administrative fee.



REFUNDS & CANCELLATIONS

There is a non-refundable registration fee of $35 per child ($25 per child when registering 2 or more siblings) which will be billed with the initial registration to secure your child’s spot in class.

There is a non-refundable recital costume fee of $80/per class for all due by October 1, 2018. There is a $5 late fee for every day the payment is late.

There is a non-refundable recital costume fee of $120/per class for all Exam Prep and Pre-Comp classes due by October 1, 2018. There is a $5 late fee for every day the payment is late.

There is a non-refundable competitive costume fee of $160/per costume for all competitive solos, duets & trios due by November 1, 2018. There is a $5 late fee for every day the payment is late.

There is a non-refundable competitive uniform fee of $160 due by November 1, 2018. There is a $5 late fee for every day the payment is late.

All competition and exam fees are non-refundable and due by Sept. 1, 2018. There is a $5 late fee for every day the payment is late.

Cancelling mid-year results in the loss of all non-refundable deposits listed above. Cancelling mid-month will result in a complete loss of the fees/payment for that particular month, herein by a refund will be issued only for the remaining full months for that year.

There are no refunds for missed classes. The Born 2 Move Dance Company reserves the right to cancel classes due to an emergency (i.e. inclement weather, power outage etc.) in which case classes will not be rescheduled or refunded. If the studio cancels class due to instructor illness, classes will be re-scheduled. Please carefully review our holiday schedule to ensure you have documented all of our scheduled holidays.

All uniform purchases & summer intensive fees are non-refundable. For health & sanitary reasons absolutely, no refunds or exchange will be issued on any uniform/hair items including shoes.



COMMITMENT & ATTENDANCE

Students are expected to commit to the full year Sept-May including the Recital Performance. Exam prep, recital choreography/moves start in September and continue through till May. Dedication & commitment to regular class schedules is essential in the growth and success of your child as a dancer, student and citizen.

Parents are strongly encouraged to schedule family vacations/getaways outside of the scheduled class time.

Understanding that there are emergencies/illness please notify the school via email of your child’s absence.

If a dancer is ill/injured, but not bed ridden, then we strongly encourage the student to come to class to observe. This will allow the dancer not to miss important choreography, corrections and exercises essential to their continued progress in class.

In fairness to dancers that regularly attend class, for dancers that miss 2 or more classes in a row, please note that the choreography will be modified in so that the rest of the class is not affected by the absenteeism of other dancers.



COMPETITIVE/EXAMDANCERS POLICY

All classes missed throughout the year must be made up. That likely means that your dancer is taking a class that is not their regular class or grade level.

Dancers missing more than 2 classes in a row, under director’s discretion, may loose the opportunity to compete or enter into exam for that season, and could altogether be dismissed from the program.

A competitive/exam dancer that has missed more than 3 competitive/exam classes throughout the year, under the director’s discretion, may lose the opportunity to compete or enter into their exam for that season.



ADDITIONAL FEES

I understand that there are additional fees including, but not exclusive to, recital tickets, costumes, make-up, tights, leotard, shoes etc. throughout the year that are normal costs associated with your dancers training and the maintenance and upkeep of their dance uniform for proper fit, look and effectiveness throughout the year.



UNIFORM EXPECTATIONS

I understand that by attending the Born 2 Move Dance Company, my dancer is required to wear a uniform for class. It is my responsibility, and that of my dancer, to ensure that the uniform has no stains, holes, rips or tears and is always clean and tidy.

I agree to purchase new tights, leotards and shoes as many times as needed throughout the year, to ensure that my dancer is meeting the uniform expectations, and that items are properly fitting my dancer for his/her own safety.

I understand that the Born 2 Move Dance Company Uniform is only to be purchased at the Born 2 Move Dance Company and that any uniform items purchased elsewhere will not be permitted in class.

I understand that should my dancer not be in uniform, that they will sit out from class and observe.



PHOTOGRAPHY & VIDEO

I understand that for the safety and privacy of all dancers at B2M I will not take photo/video of any dancers other than my own at any point throughout the year, including the recital.



STUDENT & PARENT CODE OF CONDUCT

At the Born 2 Move Dance Company we have strict policy’s regarding the behaviours of both students and parents. These policies have been put into place to ensure a comfortable, safe and bully free environment for all. Any students and/or parents failing to adhere to our Born 2 Move Code of Conduct may be dismissed from our school at any point in the season (upon which no refund will be issued) for the safety and wellness of all dancers, participants, guests and staff.



DISCOUNTS

All payments made in one lump sum prior to June 30th 2018 receive 10% off total price. Discount does not apply to rec classes.

All payments made in one lump sum after June 30th 2018 but before September 1st 2018 receive 5% off total price. Does not apply to rec classes.

All families with 2 or more children registered receive a 5% discount off total cost when paying month to month. All families with 2 or more children registered receive 10% off total cost when paying in one lump sum.

All of the discounts above cannot be combined with other offers.



BORN 2 MOVE CODE OF CONDUCT

At the Born 2 Move Dance Company our mission is to create a welcoming, supportive and encouraging atmosphere for ALL of our dancers, teachers, parents & guests. Please review the Born 2 Move Code of Conduct with your child to ensure that your dancer understands the behaviour expectations while a dancer at the Born 2 Move Dance Company.

All dancers and parents should practice the following behaviours throughout the year at Born 2 Move to ensure a safe and enjoyable experience for all. At Born 2 Move we have a zero tolerance policy for any disruptive, disrespectful and dishonest behaviour. Dancers and parents that fail to comply with the expected positive behaviour and practices may be dismissed from our school at any point in the season (upon which no refund will be issued) for the safety and wellness of all dancers, participants, guests and staff.
